This may only be a slight problem that should heal without further damage. Since you said the problem was at the top (upper portion) of the plant I would not be too concerned. If the black spot expands then you may need to do a little more. Please just watch the plant and keep in touch if the black portion expands and grows larger. Normally this sort of thing will dry up and may produce a scar but will not affect the plant further. Hope this helps. If your saguaro is growing outside in a sunny open air position you should not be overly concerned by slight spotting or other stem problems unless it seems to be extensive and a threat to the survival of the plant.
